Who Qualifies for Home Health Care?
We accept payment for services from Medicare, Medicaid, Worker's Compensation, county waivers, third party insurance, private insurance or private pay.
The following items are required for Medicare to pay for your home health care services:
- You are homebound. This means that due to your illness or injury, you are unable to leave your home without a considerable, taxing effort and absences are short and infrequent.
- You have had a recent illness or injury (or worsening of a condition) which requires the skilled care of a Registered Nurse, Physical Therapist, or Speech Language Pathologist.
- You are under the care of a doctor who has ordered the treatment or services we are providing.
What Is My "Plan Of Care" and Who Is Involved?
The Crystal Care companies involves key professionals and other staff members in developing your individual plan of care, which is based upon admission information, your physician's specific medication and treatment orders, our observations and your personal wishes.
The plan of care is designed to increase your ability to care for yourself as we provide you, your family and your caregiver with the education needed to become more independent in your health care management.
Effective pain management is an important part of your plan of care and treatment.
The plan is reviewed and updated as needed, based on your changing needs. Medical information will be provided to assist you in participating in your plan of care.
What if my service must be changed?
We are very flexible with our service and understand the care situation can quickly change. We can adjust schedules with short notice. And we do not have any "contract period" you are committed to. Care can also be established in most cases within a short time period.
What benefits do home care service offer over assisted living facilities or nursing homes?
Among other benefits, our clients continue to enjoy the privacy, comfort and security of their own homes. This provides them with a sense of independence and peace of mind.
Are your services available for the elderly in nursing homes or assisted living facilities?
Yes, we are able to provide services to care facility residents who require additional attention and/or personalized assistance.

What criteria do you use before hiring your employees?
The Crystal Care Companies is committed to providing professional, qualified and reliable caregivers. Our staff is industry certified, must have prior Home Health Aide experience and a demonstrated commitment to the well-being of our Clients. Every employee has passed a background check and screening process, allowing us to identify the best-quality caregivers in their profession. Only the most committed, reliable and compassionate candidates make it through this process.
How do you train your employees?
Once accepted, they are required to complete our own rigorous orientation program, where we continue to screen, evaluate and develop all candidates. Ongoing in-service training is also required. In addition, our Nurse Case Managers perform bi-monthly supervisory visits and carefully monitor each Client's Care Plan. Activities of the Home Health Aide are carefully recorded and documented to meet or exceed the State of Minnesota and Medicare compliance and regulatory requirements.
