Home Health Aide/Personal Care Assistants
Full-time and Part-time positions available. Must have at least 6 months experience in the health care field, CNA preferred but not required. Must have a valid driver’s license, proof of auto insurance and a reliable vehicle. Looking for strong communication skills, someone who enjoys helping other people and who wants to make a difference in someone’s life. Responsibilities, depending on each individual client’s needs, may include: performing simple procedures as an extension of therapy services under the direction and supervision of the therapist such as range of motion exercises and assistance in ambulation or exercises; personal care activities, including but not limited to, bathing, shampooing, skin care/nail care, oral hygiene, shaving, and dressing; household services essential to health care at home, including but not limited to, meal preparation/feeding, laundry, and light housekeeping; and assisting in the administration of medications that are ordinarily self-administered under the direction and supervision of the Registered Nurse (per state nurse practice laws and agency policy). Personal Care Assistants provide services to clients approved by the Medicaid program. Services provided are based on the client’s needs and are provided under supervision of a Registered Nurse. Pay range is $ 9.50 - $ 11.25 per hour based on years of experience.

Homemaker
Full-time and Part-time positions available. Must have 6 months experience with cleaning, valid driver’s license, car insurance and reliable vehicle and must be willing to travel. Looking for someone who wants to make a difference in someone’s life and create a clean and healthy living environment. Responsibilities may include: cleaning bathrooms and kitchen, laundry, vacuuming and dusting, scrubbing floors and changing bed linens, meal preparation, grocery shopping, running errands and providing companionship and socialization. Strong cleaning skills and the ability to work with a diverse clientele a must. Pay rate is a flat rate of $ 9.50 per hour.

Private Duty Nursing
Full-time and Part-time positions available. Must have a valid MN nursing license; at least one year of nursing experience in homecare is preferred.

Visit Nursing
Full-time and Part-time positions available. Must have a valid MN nursing license and must be willing to travel. Competency with peripheral blood draws, IV therapy and wound care required. Excellent assessment skills a must. Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager under the client's Plan of Care. At least one year of homecare experience is required.

RN Nurse Case Manager
Full-time positions available. Must have a valid MN nursing license. Candidates must have at least 2 years of nursing experience and have strong organization and communication skills. Position requires nurse to manage a caseload of 40 – 50 clients and provide direct care as well as supervision of unlicensed staff. Strong assessment skills a must, IV therapy, wound care and peripheral sticks knowledge needed. Determines appropriateness of referral for home care services and responds per agency policy and standard guidelines. Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services. Collaborates with physicians, other health care professionals (therapists, social services, pastoral care, and supportive services), clients, and families in developing a comprehensive plan. Directs, plans, and initiates appropriate action independently and responsively in home care situations. Makes decisions and/or recommendations that reflect consideration of immediate and long-range effects (frequency of visits, components of care plan, additional services). Monitors documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness. Provides on-call support for clients per schedule. Manages/supervises a team of RNs, LPNs, Home Health Aides, and Personal Care Assistants to provide effective and quality home care services. Identifies staffing needs and communicates this information. Participates in human resource management to achieve quality service delivery and positive employee relations. Knowledge of Medicare regulations and experience in homecare strongly preferred.

Physical Therapist
Full-time and Part-time positions available. Must be registered in the state of MN. Plans and provides physical therapy services to clients in their homes in accordance with the Plan of Care and agency policies and procedures. Performs physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care. Assesses knowledge level of client and identifies learning needs. Plans, teaches, supervises, and counsels regarding the client's physical therapy needs to promote improvement or recovery, symptom management disease process, and community resources. Evaluates the effectiveness of the teaching plan and establishes care plan for others to follow in teaching the client. Implements teaching appropriate to the client's needs and within the context of his/her environment and value system. At least one year of homecare experience is preferred.

Occupational Therapist
Part-time positions available. Must be registered in the state of MN. Performs occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care. Guides and instructs the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning. Establishes household management routines. Designs and/or orders appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning. At least one year of homecare experience is preferred.

Speech Therapist
Part-time positions available. Must be registered in the state of MN. Provides services for home care clients who have speech and language problems. Services are provided under the direction of the attending physician with the participation of the family and other members of the interdisciplinary team, as indicated. Performs speech therapy assessments, diagnostic tests, skilled treatments and ongoing evaluation for clients who are receiving service under a medically approved plan of care. Implements teaching appropriate to the client's needs and abilities. At least one year of homecare experience is preferred.
