Scope of Services
Our goal is to empower clients to develop an at-home solution that fits with their unique set of needs and desires. The Crystal Care Companies go one step further by providing Care Coordination services that can manage all aspects of a client's care and represents a meaningful alternative to facility-based assisted living.
The Crystal Care Companies team of highly trained clinicians provide an interdisciplinary approach to our home care patients. Team members include: RNs, LPNs, physical therapists, occupational therapists, speech/language pathologists, home health aides, social workers, personal care assistants, and other medical specialists.
Care Coordination services include:
- Comprehensive In-Home Assessment
- Family consultation
- Written plan of care
- Arranging and monitoring non-medical home health care
and home health nursing
- Home safety assessment and monitoring
- Arranging for durable medical equipment and home modifications
- Coordination and communication with physician
- Crisis intervention and management of complex family situations
- Assess and present options for alternative living placement
- Discharge and transfer planning
- Crisis management, prevention and intervention
- Coordination of transportation needs
- Coordination of Billing with Long Term Care Insurance
The Crystal Care Companies provide flexible options for determining the amount service that is required for you or your loved ones. The most common examples of our service are comprised in three main categories:
- Hourly Home Care which ranges from as few as 3 hours per day (minimum shift length) to up to 12 hours per day. The shifts are typically provided by a single caregiver to capitalize on the continuity of the relationship between our clients and caregivers. These services may be utilized on an as needed basis or a fixed schedule up to 24 hours per day.
- Live-In Care is charged on a daily basis versus an hourly basis, proving to be more cost-effective if this level of care is acceptable to the specific instructions of the customized care plan. For live-in care service, caregivers must be afforded reasonable sleeping accommodations, a restful night of sleep, and periodic breaks during the day.
- Overnight typically begins in the evening hours and continues into the following morning (12 hour shift) to assist with routine bedtime and early morning routines. Similar to live-in care, the caregiver must have the ability to sleep through the night.
Skilled Nursing Services
Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) are available to care for patients who are ill, disabled, recently hospitalized, or have been injured in accidents. The RN plans, develops, implements and evaluates the comprehensive care required by the home care patient. Length of service may vary from a short daily visit to monitor health signs and administer medication to continual care.

- Medication Management

- Post Operative Care
- Health monitoring
- Wound Care
- Nursing Care in support of medical plan of treatment
- Supervision of Aides and PCAs
Certified Home Health Aide and Personal Care Assistant Services
Certified Home Health Aides and Personal Care Assistants (PCA) perform those household and health-related services that are essential to the patient's well-being and necessary to prevent or postpone institutionalization. PCA services are available to clients eligible for Medicaid. Caregivers are trained to meet the specific needs of the individual client.
Our goal is to find the best possible match between aide or PCA and patient, with aides and PCAs carefully selected depending on the needs, health and preferences of the patient.
All aides and PCAs complete an approved training course and pass both written and practical tests for competence. Aides and PCAs always practice universal precautions and infection control techniques.
Aides and PCAs provide the following services under the direction and supervision of a Registered Nurse:
- Bathing and Grooming
- Toileting
- Dressing
- Walking
- Transfers (bed to sofa, sofa to bathroom, etc.)
- Activities of daily living
- Supervision of nutrition, special diets
- Self-administered medications
- Meal preparations
- Light housekeeping
